'reversed' means 'the insurance company, when they realized they were going to have to provide documentation to the state, went "ah fuck nvm" and decided to cover it' (even though their previous internal appeals process had deemed that their final decision was that they weren't gonna cover it!! huh!!). so medical necessity is 53% + 23% = 76%
