For someone who cannot get to a poll (ex: hospitalised) they can assign a proxy.

Note:

▪ The Form must be completed & signed by voter & proxy voter before being certified.

▪ the person being appointed as Proxy must get the Form certified in-person

▪ Until Oct 26, Mon to Fri 8:30 a.m. to 4:30 p.m.

▪ at the Toronto Elections office, Toronto City Hall, 100 Queen St. W.

▪ or North York Civic Centre, 5100 Yonge St..
