Incidentally the software engineer responsible left the company and, even during subsequent lawsuits, they were never found. I'm sitting here thinking there's a person out there living their life with Therac-25 hanging over their conscience... even if much of the blame can be placed on corporate priorities, marketing, and the hospitals' acceptance of reduced safety standards--should you ever buy such a machine without hardware interlock and shut off? My garage door has a safety shut off.

Therac-25 was a machine used for radiation therapy and highlighted the foolish enthusiasm some people feel for uniting profit motive and engineering arrogance. The machines were meant to treat disease but the removal of hardware interlocks (!!), various bugs, integer (bit?) overflow, and race conditions (have you *seen* how medical people click through menus??) meant that several people were killed and others burned by radiation. The company slow walked investigation.
